CONDITION :

Unusually it is printed on one complete sheet (usually 3-sheets comes in two parts), Additional horizontal fold lines, approximately ten pinholes near the top left corner (several have torn together and some a several inches away from the corner), approximately four pinholes near each of the other corners and near the side edge centers (again some have torn and some are a few inches away from the corners), several creases from the side edges up to the vertical fold lines nearest the edge, some creasing around some of the fold intersections, two pinholes on the title block, tiny tear at a couple of the fold intersections, Otherwise Very Good.
